One waiver per participant **Participant Information & Waiver** Please complete all required fields and submit the form below. ## SIMULATION ZONE PARTICIPANT WAIVER, RELEASE & MEDIA CONSENT This Waiver applies to participation in activities at Simulation Zone Inc., including but not limited to sim racing, motion-enabled racing simulators, flight simulators, Omni Arena VR, virtual reality activities, use of related equipment, and participation in events or other activities on the premises. ### 1. Acknowledgement of Risk I understand that participation may involve risks including, but not limited to, dizziness, nausea, motion sickness, disorientation, loss of balance, falls, collisions, sudden simulator movement, physical strain, emotional distress, contact with equipment, and injury caused by misuse, equipment malfunction, or failure to follow instructions. I understand that motion simulators, VR systems, steering wheels, pedals, seats, headsets, and other equipment may move or create physical sensations that may affect me differently than expected. ### 2. Voluntary Participation & Assumption of Risk I voluntarily choose to participate and knowingly assume all risks associated with participation, whether known or unknown, foreseeable or unforeseeable, to the fullest extent permitted by law. I understand that I may stop participating at any time. ### 3. Health & Safety Declaration I confirm that I am physically and mentally able to participate safely. I will not participate if I am experiencing dizziness, illness, injury, intoxication, or any condition that could make participation unsafe. If I feel dizzy, nauseous, disoriented, unwell, or unsafe at any time, I will stop immediately and notify staff. ### 4. Rules, Staff Instructions & Equipment Use I agree to follow all posted rules and all instructions given by Simulation Zone staff. I will use all equipment only as intended and will not intentionally misuse, damage, move, disconnect, alter, or interfere with any simulator, VR equipment, controls, cables, seats, motion systems, or other property. Simulation Zone may stop my participation at any time if staff believes my conduct is unsafe or disruptive. ### 5. Release and Waiver of Liability To the fullest extent permitted by law, I release and waive claims against Simulation Zone Inc., its owners, officers, employees, contractors, agents, affiliates, landlords, and representatives (collectively, the “Released Parties”) for injury, illness, property damage, loss, or other harm arising out of or related to my participation, except to the extent such release is prohibited by law. ### 6. Indemnification To the fullest extent permitted by law, I agree to indemnify and hold harmless the Released Parties from claims, damages, costs, or expenses arising from my conduct, misuse of equipment, violation of rules, or actions that cause injury or damage to another person or property. ### 7. Minors If I am signing on behalf of a minor, I represent that I am the minor’s parent or legal guardian and have authority to sign this Waiver on the minor’s behalf. I accept responsibility for the minor’s participation, conduct, compliance with staff instructions, and use of equipment. ### 8. Photography, Video, Audio & Promotional Media Release I understand that Simulation Zone Inc. may photograph, video record, and/or audio record guests while participating in activities or while on the premises. I authorize Simulation Zone Inc. to use, reproduce, edit, publish, display, and distribute photographs, video recordings, audio recordings, and other media containing my image, likeness, or voice for Simulation Zone’s website, social media, digital advertising, printed advertising, promotional materials, and other marketing purposes, without additional approval or compensation. If I am signing on behalf of a minor, I represent that I am the minor’s parent or legal guardian and grant the same permission on the minor’s behalf. ### 9. Governing Law & Venue This Waiver shall be governed by the laws of the State of Illinois. Any legal action arising from this Waiver or participation at Simulation Zone shall be brought in a court of competent jurisdiction in Lake County, Illinois, unless applicable law requires otherwise. ### 10. Acknowledgement & Agreement I confirm that I have read and understand this Waiver and Release, understand that I am giving up certain legal rights, and voluntarily agree to its terms. I understand that my electronic submission and agreement to the waiver checkbox are intended to serve as my signature and acceptance of this Waiver.
